An early-pipeline rare disease asset needed more than a traditional patient journey map. The brand team’s ambition was to use deep human understanding to simultaneously guide clinical trial design and shape commercial strategy before the competition had established the market narrative.
Rare disease patients are hard to recruit and emotionally vulnerable, requiring research methods that honor their experience without exploiting it. The asset was early-stage, meaning there was no existing brand or market frame to react to. And the brand team needed insights that could drive alignment across clinical, medical affairs, and commercial functions — not just inform one team’s planning.

The S+R Approach
Shapiro+Raj designed a multi-method qualitative immersion that blended the richness of deep human contact with the analytical power of Stella AI.
The foundational layer was digital ethnography monitoring patient communities, caregiver forums, and disease-specific social spaces to build a textured understanding of the lived experience before any primary contact was made. This grounded the research team in authentic patient language and prioritized the most resonant emotional territories.
In-depth interviews and in-home immersions brought researchers into the daily reality of patients and caregivers, capturing not just stated experiences, but observed behaviors, environmental context, and moments of emotional truth that traditional interview formats rarely surface.
All research data was uploaded into Stella, which was trained on transcripts, reports, and secondary sources to build three humanized patient archetypes, each a living Stella Avatar able to synthesize the patient experience, respond to new questions in human voice, and react to new stimuli.
The final phase was a co-creative activation workshop built around the Empathy + Clarity = Action framework — combining experiential immersion exercises with SPICE design thinking to translate empathy into concrete strategic action.

The research delivered what the brand team described as being “way ahead” for an asset at this stage of development. Clinical teams received clear endpoint and PRO guidance grounded in patient voice. The education strategy — including a direct-to-patient awareness concept and partnership frameworks with national patient advocacy organizations — was developed from insight rather than assumption.
The Stella avatars remained active post-study, allowing the brand team to continue asking questions of their patient archetypes as new data, new stimuli, and new strategic questions emerged — extending the value of the research investment indefinitely.
